Yoga with Meghan Morrison Liability Waiver

 

 

By signing this waiver, I hereby agree to the following:

That I am participating in a Yoga Classes offered by Meghan Morrison, during which I receive information and instruction about a healthy and safe practice. I recognize that these classes and/or workshops may require physical exertion, which may be strenuous and could result in physical injury, and I am fully aware of the risks and hazards involved.

 

I understand that it is my responsibility to consult with a physician prior to and regarding my participation. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation enthuse classes and/or workshops.

 

I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the program. I agree to inform my instructor/teacher (Morrison) of any physical limitations, physical discomfort and/or injuries before or during class, and I take full responsibility for nondisclosure.  

 

In further consideration of being permitted to participate, I knowingly, voluntarily and expressly waive any claim I may have against Morrison for injuries or damages that I may sustain as a result of participating.

 

 

 

Media Release Form

 

I give my permission to Meghan Morrison and any media in attendance to publish any photographs or video taken during this class to be published on any and all forms of media, worldwide.

I hereby agree that I will not bring or consent to others bringing claim against Morrison on the grounds that anything contained in the property, or in the advertising and publicity used in connection herewith is defamatory, reflects adversely on me, violates any other rights whatsoever, including, without limitations, rights of privacy and publicity. I hereby release Morrison, its creators, employees, and volunteers and assigns from and against any and all claims, demands, causes and actions, suits, costs, expenses, liabilities, and damages whatsoever that I may hereafter have against Morrison.

 

Morrison shall have the right to assign its rights hereunder, without my consent, in whole or in part, to any person, film, or corporation.

 

 

 

I have read the above release waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
